Project Description
Centralized Intake is the foundation of the homelessness response system in Polk County. Over 2,000 individuals a year who are experiencing homelessness or are imminently at risk of homelessness are served by Primary Health Care’s Centralized Intake program. Through compassionate, in-person conversations, staff members assess each individual’s unique situation and needs to connect them with available resources. By prioritizing the needs of individuals experiencing homelessness, our most vulnerable community members can access the limited resources available.
In addition to connecting individuals and families to community resources, Centralized Intake provides access to showers and restrooms, a clothing closet, a place to receive mail and phone calls, support with obtaining vital documents, and transportation support to safe destinations for relocation.
Centralized Intake is a critical program that meets a host of unique needs for those experiencing homelessness.
